Community Based Services

Supportive Services, Crisis Intervention and Case Management for families, seniors, children and individuals with significant disabilities.

  • Community Based Services provide case managements and support for over 2,500 abused and/or neglected children across Texas each year.
  • Staff provide essential supportive services to over 150 low-income seniors, shut-ins, and adults with disabilities each year through regularly scheduled in home visits.

Program Services:

  • Family Care Services include:
    1. Arrange for and transport clients to medical appointments
    2. Assist clients with financial management
    3. Locate and contact resources to obtain emergency assistance
    4. Assist clients with applications for Food Stamps, Social Security and other benefits or social services
    5. Visit clients to monitor the living situation and provide feedback to the case manager, family and/or guardian
    6. Obtain and deliver food, medication and/or other items that are needed to provide for the health and safety of the client
  • Case management, foster home studies, and adoptive screenings for abused and/or neglected children across 28 Texas counties.

Performance Outcomes:

Increase and promote mental health and social services to underserved children, youth, adults, seniors and families through:

  • Providing over 2,500 foster home studies, and adoptive screenings to reduce the abuse and/or neglect of children.
  • Improving the quality of life for low-income seniors, shut-ins, and adults with disabilities through supportive services; over 150 clients served in 2014.
  • Preventing homelessness for adults, children and families through homeless prevention services.
